Changes to Surrey dementia support services in Guildford and Waverley
From 1 December 2023, dementia care and support will be provided by Neighbourhood teams who focus on local areas in Guildford and Waverley.
These teams can provide any advice and support needed by patients, carers and families including linking in with other services, where this would be helpful. The teams consist of professionals who can connect patients to activities, groups, and services in the local community (social prescribing link workers), Admiral Nurses who provide more tailored support to the patient and carer, community matrons, social workers and mental health practitioners. Patients can contact their GP practice and a Care Co-ordinator will be able to help them further.
