Pilot project launches post-discharge welfare calls to prevent readmissions

An innovative pilot collaboration project being run by Guildford and Waverley Health and Care Alliance, the Royal Surrey and Mole Valley District Council has launched to offer follow-up welfare telephone calls for older patients discharged from hospital. 

Anyone aged over 50 and living in Guildford and Waverley who has had a short stay in hospital and is being discharged with no further clinical follow up, no new care and support needs, will now receive a ‘check-in call’ if appropriate, offering them additional support if needed
